ZyyandCodex 02177af921 fix(accounts): refresh OAuth tokens while scheduling is disabled (#291) ...

Separate scheduling availability from OAuth candidate selection and credential
state writes across OpenAI, xAI, and the shared account store. Cover disabled
refresh, terminal failures, and in-flight scheduling changes with regression tests.
