ZyyandCodex 12ecbf574a fix(accounts): fill missing plans from provider quota snapshots ...

Use explicit plan types from OpenAI quota snapshots to fill missing or
unknown account plans before building account and dashboard display names.
Preserve known plan subtypes and reuse cached quota without extra requests
or changes to persisted credential facts.

Cover quota projection and account list/detail fallback behavior, and
document the backend display contract.

Validation: gateway-admin, OpenAI, and xAI tests (1049 passed), strict
Clippy for affected backend crates, gateway build, and diff whitespace
checks passed. A real account JSON import and upstream quota refresh
confirmed free/Free in list, detail, and cached quota responses.
