Forward numeric and extended reasoning effort values and preserve standalone endpoint business headers.

Retain business metadata and original WebSocket text through provider, middleware, and client delivery. Validate rewritten plugin output without rejecting unchanged heartbeats or opaque messages.

Add HTTP, WebSocket, and plugin process regressions for the passthrough boundaries.
