ZyyandCodex 7b05b14a2b fix(openai): narrow model rejection cooldown detection ...

Require the official model_not_found code and supported status codes for
account-model cooldowns. Treat unsupported_value as a terminal request
error and preserve account eligibility across HTTP, SSE, and WebSocket.
