* fix(openai): preserve session affinity for managed upstream adapters

* test(openai): avoid coupling wire invariance to adapter lookup count

* fix(openai): preserve plugin control over adapter request identity

Keep affinity and turn aliases tied to the admitted request before attempt middleware. Pass plugin rewrites to the adapter without native identity revalidation.
