3 people d0180b74b3 fix(openai): refresh expired quota windows after reset grace (#170) ...

Refresh non-exhausted accounts after the quota reset grace period while preserving observed usage and exhausted-window attribution. Keep periodic checks for exhausted accounts to detect early upstream resets, and cover refresh timing, throttling, and display regressions.
