ZyyandCodex d700df8a4f fix(openai): separate transient retry exhaustion policies ...

Retry ordinary 503 server errors up to five times on the same account with capped backoff and positive jitter. Preserve upstream Retry-After and return the final error without rotating accounts; retain rotation for explicit capacity rejections.
