ZyyandCodex de599ed768 fix(openai): apply timezone overrides without content kinds ...

Recognize complete Codex environment contexts when custom providers omit
content classifications, while keeping explicit classifications authoritative.

Cover HTTP and WebSocket overrides and proxy location precedence, and
document timezone and client privacy boundaries.
