ZyyandCodex e830b2ef48 fix(openai): isolate account context and scope websocket interrupts (#298) ...

Filter account-owned routing and identity context at API and Provider boundaries. Bind WebSocket interrupts to the active response on its original account and socket, preserving valid continuations.
