ZyyandCodex faf50c28ad feat(usage): unify request observations and upstream performance 
Consolidate request diagnostics into typed request_observation_json with
migration 0025, preserving historical facts and query constraints.

Use upstream response timestamps for output throughput, align TTFT event
boundaries, and surface official timing metrics in the latency popover.
