ZyyandCodex 9db8528758 fix(metrics): align TTFT and throughput with sub2api ...

Count non-preamble Responses events for first-token timing while preserving
semantic output boundaries and request-level timing origins. Calculate
output throughput over the complete request duration in usage rows and
aggregate queries.
