ZyyandCodex 6f7a50b1b6 fix(scheduling): rotate smart selections among similarly scored accounts ...

Score load, remaining quota and first-output latency on fixed scales so
small differences do not monopolize unbound requests. Rotate accounts
within a narrow score tolerance in stable ID order using the existing
cursor, preserving session affinity and availability checks.

Cover small signal differences, changing score order, material advantages
and quota invalidation after credential rotation.

Validation: workspace tests (1844 passed, none ignored), rustfmt, and strict
workspace Clippy with all targets and features.
