ldm0 ba803e7b90 fix(xml): report malformed attributes while preserving namespace parsing ...

Check strict lexical XML errors before xml5ever tree construction. Cover six malformed attribute/name forms both alone and in a valid XHTML prologue, plus distinct xml:lang/lang namespaces.

Validation: cargo fmt --all; cargo clippy --workspace --all-targets --all-features -- -D warnings; cargo nextest run --no-fail-fast (20283 passed, 18 skipped). Release v3 and five browser regression rounds. Full WPT recorded separately.
