HistoryItem::ToolsDeclared { names } 记录每轮工具名称快照，
变化才记不逐轮重复。纯 transcript 事实：模型侧仍由
request.tools 携带定义，history_to_messages 跳过回灌。
is_system_note 白名单纳入；compaction 估算/摘要覆盖；
UI load_history 忽略不产生气泡。
