Neil 703e6aa9b6 perf(relay): stop snapshotting the whole pending-PTY map every drain tick
flushPendingOutput allocated one [id, pending] tuple per live session via
Array.from(entries()) on every 8 ms tick, then consumed at most
PTY_OUTPUT_FLUSH_MAX_WRITES (2) of them. Capture just that bounded prefix
instead. Cost goes from O(sessions) to flat: at the 50-session cap,
750 -> 98 ns/tick.

Exact behavioral equivalence, including under a synchronously re-entrant
sink: the send body has no skip path, so the legacy loop could never reach
a third snapshot entry, and the prefix is captured before the first send so
re-entrant mutations see what a whole-map snapshot would have frozen.

Adds a differential test that replays the pre-change snapshot loop as an
oracle and asserts frame-for-frame equality, including legacy's
append-mid-tick drop quirk so this stays a no-behavior-change optimization.
