Brennan Benson 1818553aa6 fix(codex): keep a dismissed restart prompt dismissed across app restarts ...

"Keep old account" cleared the renderer notice but left the on-disk launch
record, so the startup sweep re-raised the same prompt — and re-blocked that
pane's terminal input — on every subsequent launch. Dismissing now drops the
record through a new codexAccounts:forgetStalePanes handler, matching the
registry's own rule that an unrecorded PTY is never reported.

Also stubs both stale-pane methods in the web preload shim, whose `as never`
cast hid the gap; the sweep rejected with a TypeError there instead of
reporting no stale panes.
