* fix(ssh): never start a hidden orcad shell for a pane the legacy relay bridge serves

* fix(ssh): a skipped bridged activation still supersedes an earlier tab's pending focus

* fix(ssh): a click on a whole mirrored tab never activates its bridged pane on orcad

* fix(ssh): load the bridge pane list lazily from tab activation
