Neil 8dcbd624ce fix(diffs): pull the add-note gutter button out of the line-number column ...

Pierre pushes its gutter "+" right with margin-right: calc(-1lh + 1ch), which
overlapped the line-number column by 14px. There is no CSS variable for it, so
override it through unsafeCSS with the data-attribute-only selector Pierre's
styling docs sanction. Measured in-app: overlap 14px -> 0px.

Also makes the diff surface focusable. Monaco's editor could hold focus, so
Cmd+F reached it; a plain div cannot, and a read-only diff has no
contenteditable, so the find shortcut never fired. This is necessary but not
yet sufficient - Cmd+F still does not open the panel on read-only diffs.
