* Fix cancellation result fences and exercise host proofs

* Fix input finalization type contracts

* Remove obsolete Portuguese permission description

* Preserve proven Stop outcomes during input finalization

* Preserve input handover proofs with session-ending Stop

* Preserve pending input recovery before lifecycle cancellation

* Assert one recovery card across overlapping close requests

* Preserve unwritten inputs across shared-agent Close preparation

* Narrow cancelled dispatch to its rejected outcome

* Import Codex app-server types from their colocated modules

* Use standard Ask policy for fresh Codex chat input

* Deliver applied Codex permission reports to the host
