* fix(serve): drive hook titles and first-work rename without a window

Headless orca serve never opens a window, and both the synthetic agent titles
(Hermes readiness) and first-work branch/workspace rename were driven only from
the window's hook listener. Subscribe both to the hook store directly at
startup, and feed synthetic frames to the runtime tracker before the window
check, so they run in serve and desktop alike.

* fix(serve): narrow the hook event for the rename entry

* fix(serve): a new turn's working title reaches the tracker unseen; keep spinners across window reopen
