Jinwoo-H b082443e1f Merge origin/main (PR #18902 typed dispatch refusals) into orchestration-v3 ...

Ported the typed refusals onto v3's file layout: runs/dispatch-methods.ts, gates/gates.ts,
the dispatch store, and worker-dispatch-start. Dropped v3's inject-rejection-message module
in favour of the shared refusal contract. The receipt-code table lives in
references/recovery-and-cleanup.md since the kernel routes refused starts there.
