Brennan Benson d6df085e6f fix(rate-limits): state the unreadable-response rule once and make every provider express it
A response Orca cannot fully parse is unreadable; unreadable never overwrites a
good snapshot and never hides the surface that would show something is wrong.
Three clauses, in unreadable-usage-response.ts: prove the body is an object
before reading a field off it, treat a present field of the wrong type as a
failed read rather than an absent one, and settle 'error' — the only verdict
that neither overwrites (ok) nor discards-and-hides (unavailable).

- claude-oauth-usage-request: a JSON null body threw a TypeError out of
  `data.five_hour`, which classified as 'unknown' and put the engine's own words
  in the tooltip instead of the parse-failure copy this PR added.
- grok-fetcher: a present config carrier of the wrong type (`[]`, `"invalid"`)
  was truthy, produced no window, and landed on 'unavailable' — discarding the
  last good snapshot and hiding the chip, the exact harm this PR set out to
  prevent. The monthly fallback view read the same carrier the same way.
- minimax-fetcher: same shape as Claude's, one provider over — reading
  `base_resp` off a JSON null threw and filed as a network failure.

A carrier-less 200 stays 'unavailable' for Grok: that is the documented
"this plan has no weekly credits" answer and it predates this PR.
