feat: iOS session handling survives network blips and explains sign-outs - token refresh only drops the session on definitive rejections (network errors rethrow and keep tokens), a missing token now kicks cleanly to sign-in instead of hanging, and the auth welcome step shows an amber session-expired notice when the app signed the user out
