Renamed deprecated type aliases following rmcp library update:
- CallToolRequestParam → CallToolRequestParams
- InitializeRequestParam → InitializeRequestParams
- PaginatedRequestParam → PaginatedRequestParams
Also added required `meta` field to ClientInfo and CallToolRequestParams.
