Guilhem LemouelandClaude Opus 4.8 3be0369e54 fix(ai): never report an incomplete lint as clean

Addresses review findings on the headless linter: a TypeScript worker that
never initialized, an LSP pull that failed, and an LSP push that timed out
were each read as "no issues". Each now surfaces the checker as unavailable
so the tool reports the result as inconclusive rather than clean. The
collapsed status header reflects it too. Also guards the app-file setValue
against overwriting an attached editor buffer.
