When user update we `clean` the `USER_RUM_TOKEN` and in the next we
never set `{DEFAULT_ORG}/{token}` back to `USER_RUM_TOKEN`, so the RUM
ingestion report token not found for root user.
The solution is we don't `clean` the tokens, just `remove` the token
which already invalid. also will add `{DEFAULT_ORG}/{token}` if it is
root user when we fetch data from db.
