Three gaps, all in the same shape: correct code with nothing holding it, or a guarantee that stopped short of the branch that matters. The legacy lane's repair sat below the blank-source early return while its comment claimed to be unconditional. A user whose ~/.codex/config.toml is absent or 0 bytes — the cloud-synced case the code itself calls out — kept a world-readable legacy config and backup indefinitely, and that lane is the only repairer of the retired home. The call now precedes the return. None of the legacy lane's mode guarantees had a test: dropping the write mode, deleting the repair, or both, left the suite green. They are pinned now, including the absent-source and 0-byte cases that Finding 1 was about. The bigger gap is which branch was tested. Codex materialises the bundled marketplace directory itself, so it never exists on the first mirror pass — the conditional rewrite correctly declines there and can only ever land on a later pass, through the merge branch. Both lanes' merge branches were unpinned, so the only path by which this fix reaches a user was the one path no test covered. Each lane now has a two-pass test that declines first and re-roots second, which is the real-world sequence. Also records what the existence probe does and does not promise: it is a presence test, and for mcp_servers.*.env.CODEX_HOME the target is the runtime home itself, so that key is re-rooted unconditionally. The no-worse-than-today guarantee covers marketplaces.*.source alone.
