A stalled write-back returns before the mirror runs, so the repair below never executed on that path: a runtime config already at 0644 stayed there, with its token-bearing .bak beside it, for as long as the stall lasted. Not a regression — the merge base is 644 here as well — but a cell this PR's own goal did not reach, which is what made "the repair runs unconditionally" not quite true. Placed inside the stalled branch rather than above the early return. Above it would also change the indeterminate return further down, which has its own refusal semantics; this way the only behaviour that moves is the one cell. The stall needs the ~/.codex DIRECTORY non-writable, not the file — promotion writes via temp-file-plus-rename, which needs directory permission. A read-only config.toml does not stall, so the test sets 0o500 on the directory.
