* refactor(agent-status): tighten the restored-status sweep after review

- reword preserveResumeIdentity to the real rule: the pane survives to resume into
- one isLocalHookConnectionId predicate for the sweep, removed-worktree cleanup, and runtime PTY sources
- the startup wrapper is file-private with a distinct name; probe catch simplified
- stale 'liveness reap' wording in the Claude roster comment and test
- tests: the Claude roster case is asserted through a later turn settling, and a hydrated
  resume remnant is pinned as never a candidate
